Evaluation and management of premature ventricular complexes.

چکیده
Premature ventricular complexes (PVCs) are a common cause of palpitations. Related symptoms include difficulty breathing, chest pain, fatigue, and dizziness. PVCs are also commonly detected incidentally on electrocardiography, outpatient ambulatory monitoring, and inpatient telemetry. Treatment goals include palliating symptoms, restoring cardiac function if affected, and preventing progression to tachycardia-related cardiomyopathy if the PVC burden is high, even in patients without symptoms. Responses to caffeine reduction, cessation of stimulants, and stress reduction are inconsistent. Aerobic exercise is rarely effective and can sometimes exacerbate PVCs.

Detection and Classification of Heart Premature Contractions via α-Level Binary Neyman-Pearson Radius Test: A Comparative Study
The aim of this study is to introduce a new methodology for isolation of ectopic rhythms of ambulatory electrocardiogram (ECG) holter data via appropriate statistical analyses imposing reasonable computational burden. First, the events of the ECG signal are detected and delineated using a robust wavelet-based algorithm. Then, using Binary Neyman-Pearson Radius test, an appropriate classifie...
متن کاملCatheter ablation of a monofocal premature ventricular complex triggering idiopathic ventricular fibrillation.
A 62 year old man was admitted for evaluation of recurrent episodes of syncope. A surface ECG showed frequent repetitive premature ventricular complexes of right ventricular outflow tract origin. Ventricular fibrillation was inducible by programmed electrical stimulation but otherwise cardiac evaluation was unremarkable. A diagnosis of idiopathic ventricular fibrillation was made and an implant...
متن کاملEvaluation of the efficacy of flecainide acetate in the treatment of ventricular premature contractions.
The efficacy of flecainide acetate in suppressing ventricular premature contractions in 14 patients was evaluated in a double-blind, cross-over, placebo controlled, randomized and balanced study. Each treatment period was 2 weeks followed by a 3-4 d placebo washout period and the study lasted 5 weeks. Flecainide was given in a dose of 200 mg twice daily. A significant reduction in the total num...
متن کاملPremature Ventricular Complexes and Left Atrial Appendage Dysfunction - Another Head on a Many-Headed Hydra ?
Premature ventricular complexes (PVCs) in the absence of structural heart disease are often, but not always benign. They may require treatment for symptoms produced by the PVCs themselves or for intermittent sustained ventricular tachycardia. Even in the absence of symptoms, frequent premature ventricular complexes may lead to the development of left ventricular systolic dysfunction as a form o...
متن کامل[Initiation modes of spontaneous monomorphic ventricular tachycardia in patients with Chagas heart disease].
INTRODUCTION AND OBJECTIVES We sought to demonstrate the mode of spontaneous onset of sustained monomorphic ventricular tachycardia in patients with Chagas' cardiomyopathy. METHODS We studied 222 stored electrograms in 14 patients with Chagas cardiomyopathy and spontaneous monomorphic ventricular tachycardia treated with a cardioverter defibrillator. Premature ventricular complexes before ven...
